Medication Authorization Form

If your child must take medication during the school day, the medication authorization form below, must be filled out and signed by your child's doctor. This includes, but not limited to, any over the counter medication such as Tylenol, Ibuprofen, cold medicine, cough drops, etc.
Please do not bring the medication to the office unless the form has been completed.
