Activities

Will your child participate in any extra curricular activites this year?

If your child plans to participate in any extra curricular activities offered by Rankin School, there are two forms that must be filled out by each student and his or her parent/guardian.

The "Parent & Student Agreement to Participate in Extra Curricular Activities" form must be signed by the parent and student wishing to participate.  This form includes all activities offered and will remain on file for the entire school year.  By signing this form it does NOT obligate a student to the activity.

The "Authorization for Medical Treatment and Certificate of Physical Fitness" form must be filled out and signed by the parent/guardian.

The original two-sided form will remain in the office for the entire school year.  Copies of the Medical Release/Certificate of Physical Fitness will be given to each coach/sponsor of the activity your child wishes to participate in.  It is EXTREMELY IMPORTANT that you keep all information regarding your child's health, contact names and phone numbers current!  You may contact Scott King or the school office with any changes.
