You should be receiving a registration packet in the mail during the week of July 27, 2009. If you do not receive one, please call the school office at 346-3182.
Please complete all information and bring to registration. Below is the letter you will be receiving in the mail.

Enclosed is your registration packet. Please complete the registration forms thoroughly. Student enrollment forms for all current and new students must be filled out due to new state required information. If your child is required to have an exam, a form will be included in your packet. Bring all completed forms and any other documents listed below with you on registration day. We look forward to seeing you!

Tuesday, August 4, 2009 1:00 p.m. – 5:00 p.m.

Incoming Kindergarten students and those new to the District are to bring proof of residency documents, child’s original, certified birth certificate (the one with the raised seal - not the hospital copy), current physical examination including vision exam (Illinois state law effective 01/08), current immunization records and dental exam. (Disregard this if you brought the information in during Kindergarten screening.)
Kindergarten, 2nd grade and 6th grade studentsare required to have a current dental examinationrecord.
6th grade students are required to have a current physical examination.
Book Rental Fees/Other Fees: The book rental fee is set at $75.00 per student with a $10.00 discount if paid before 9/1/09. The book fee also covers magazine subscription costs to classrooms such as Weekly Reader and Science News. There is a $2 gym lock rental fee for all Jr. High students. You may deposit money into your child’s meal account at this time. Breakfast is $1.30 and Lunch is $1.90. You may also order a yearbook at the pre-sale price of $15.

Free/Reduced Meal Application is enclosed. If you would like to apply for free/reduced price meals please complete the form and bring with you on registration day. For those of you who are reported as “Directly Certified”, an application is not necessary. We will include your “direct certification” letter with your registration packet.
The Book Fee Waiver/Reduction form is located on our website, included in your Student & Parent Handbook and will be available during registration. You MUST fill out this form to receive a waiver regardless of your free/reduced meal status.
Baseball/Softball for 5th – 8th grade students: Softball tryouts begin Monday, July 27 and Baseball tryouts begin Monday, August 3, 2009. You MUST have a sports physical (6th grade students may use their school physical) and all required forms to attend practice. Visit our website (under Activities) for schedules. Please dress in appropriate attire, bring ball glove and water. Reminder: athletic physicals are good for 12 months and are required for all sports.
First day of school for students is Tuesday, August 18, 2009 with dismissal at 11:30 a.m. (No meals will be served)
Dismissal on August 19-22 will be at 2:15 p.m. (meals beginning August 19)
