The Agreement to Participate form MUST be filled out and signed PRIOR to the activity starting. This form includes all activities offered by Rankin School, therefore, only has to be filled out once. It will remain on file for the entire school year. (Having this signed form on file does NOT obligate the student to any activity.)
The Authorization for Medical Treatment form MUST be filled out and signed for EACH activity your child is participating in.
Here are the two forms to be printed out, signed and returned to the coach/sponsor ASAP.
