The Agreement to Participate & Authorization for Medical Treatment is a TWO-sided form. Both sides must be filled out and signed before your child can participate.
The original will be kept in the office. It will be photocopied for the coach/sponsor of each activity your child participates in.
IT IS CRUCIAL YOU KEEP THE INFORMATION CURRENT!!! You must notify Mr. King with any changes to your child's health so it can be noted on the medical form! You must notify the school office with any changes to your contact/emergency contact information so it can be noted on your form and a new copy given to the coach/sponsor!
Please note: Having the signed forms on file does NOT obligate your child in any way.
