Please fill out and return the downloadable form below if your child will be taking medication during school.
If the medication is an "over the counter" type, (tylenol, ibuprofen, etc.) it is not necessary to have your physician fill it out. However, if your child requires a prescription drug to be taken while at school your physician will have to fill out the form.
